The care providers we refer offer services that help make living at home easy and safe for our clients. We will match your specific skills with the client's needs that can include Companion Care, Personal Care, Live-In Care, 24-Hour Support, Alzheimer's and Dementia Care.

Application Requirements

 

Home Health Aids

  • CNA License in Ohio (or HHA certificate - 40 hour minimum)

  • Valid CPR , First Aid and AED Certification

  • Level 2 background screening on file with ACCA – State of Ohio

  • Free of Communicable Diseases (dated within the last 6 months)

  • HIV/AIDS Certificate - 1 Hour

  • Alzheimer's Training Certificate - 1 Hour

  • Self-Administration of Medication Certificate - 2 Hours

  • Professional Liability Insurance - recommended

  • Driver's License

  • Social Security Card

  • Auto Insurance & Vehicle Registration - You must have your own transportation

Home maker, Companion and Sitter

  • Level 2 background screening on file with ACCA – State of Ohio

  • Two work references

  • Social Security Card

  • Professional Liability Insurance - recommended

  • Current Ohio Driver’s License

  • Current Auto Insurance

  • Current Vehicle Registration – You must have your own transportation
